3CX phone for Windows call "cancelled" after 3-seconds

I can make internal ext-to-ext calls and external ext-to-ext calls to my iPhone from my 3CX v5.01 windows client, but whenever I try to make an outbound call to the PSTN, the call rings once, connects, and then I hear the message "cancelled" and the call is disconnected. The cancelled message is from the PBX. My inbound and outbound rules work fine on all of the other phones. Does anyone have a way to fix this?

The 3CX logs will show how it is handling the call and , perhaps, why it is ending.
