When accessing a database through an integrated security connection string, which user will the system attempt to use? The user account for one of the 3cx windows services?

If I recall, you have to setup your SQL Server for multimode authentication (Windows/Domain + SQL Server Auth) and/or create a DSN and specify it in the VAD program.

You can not create the connection string manually. For SQL Server you need to specify username, password, database name and server, and it uses SQL Server authentication. If you need to use Windows Integrated Authentication with SQL Server you can do it using ODBC, and configure the DSN to do it.
