    Are you POSITIVE your IP is being registered at your VOIP provider correctly? This is exactly the same issue I had, and that was the problem. And, FWIW, dialing 888 from an internal extension disconnects me as well.

    From my tests, the caller ID is controlled by your Skype subscription account settings. I haven't been able to send out anything other than what's configured there (i.e. check your settings at skype.com) Regarding outbound calling, sounds like you may have "Allow outbound calls on this line"...
