Firstly, hide your public IP, to show it is just asking for trouble.

Are you using the same extension number as the working lap top phone is?

If not, did you remember to allow Ext 1001 to register remotely?

Are you trying this from behind the same (remote) router as the laptop?

There is no "Local SIP port" showing, why?

Do you see an attempted registration in the 3CX log?

Does it work when you are on the same LAN "In the office"?

This is the port number that you wish 3CX to contact your phone on. Since you are using this at remote locations it is probably best to use something with less chance of a conflict, like 5064, or a bit higher. Port 5090 is usually just used to contact the 3CX PBX when using the tunnel feature, which I don't believe is yet implemented on the iphone. Even if that is missing there should still be something showing in the 3CX PBX log.

I'm also wondering if the public IP of your 3CX server location needs to go into the Proxy setting (in the first screen).
Never mind, i just checked my Android phone and that is left blank.

If you are not seeing any registration attempt, then the registration requests are going to the wrong place, or not going out at all. Since your laptop works, I have to assume that your router/firewall configuration , at the PBX end is correct.
