External extension can hear anything

    I am using Grandstream 2000 phones with 3cX Pro.
    I have 1 Grandstream 2000 phone at a remote location.
    The server is on the DMZ and also ports have been forwarded.
    The remote phone registers and can dial any other phone on the internal network and others can hear him but he can not hear anyone talking to him. Also, the other 4 phones on the local network can't dial the remotes extension. There is complete silence and then it goes to voice mail.
    ISP Verizon FIOS 10/2
    Logs don't show much at all.
    Any suggestions?



    Ports opened and forwarded on the router:
    UDP 5060-5061 -> 5060-5061
    UDP 3478 -> 3478
    UDP 9000-9007 -> 9000-9007
    TCP 5090 -> 5090
    TCP 5060-5061 -> 5060-5061
