Firewall Checker - Getting this error- any ideas why?

Just wondering if anyone knows what this error means? (assume it is a port forwarding issue) But could use more specifics.
Couldnt find any specifics under the URL they mention.

What make/model router are you using 3CX behind? I'm going to assume that it is doing some port translations. There may be some settings to disable this if it isn't enabled for some specific reason.

I am using a router package (operating system) called Pfsense.

The modem/router that PFsense is connected to is a Dlink DSL2750b.
Have fwd'd ports 5060, 5090 and rtp 9000-9014 to the Wan port of PFsense.

I am trying to troubleshoot my config within Pfsense , however it'd be helpful to know what exact problem the 3CX (firewall) is reporting so I can investigate what is not set up correctly within PFsense.

The 3CX PBX and all connected equipment/handsets works fine when connected by tradional means to the Dlink DSL2750b, so this issue is definitely due to my config within PFsense.

Any ideas of what the error is that the 3CX firewall checker is saying?
 
Sounds like double NAT. You have the Dlink dsl/router/firewall in front of the pfsense router/firewall. The port forwarding arrangement is hidden because of the two device arrangement. Why both?
 
Looks like your having NAT issues you want to make sure your router has NAT disabled.
