Hi, I see from each extension's forwarding rules under Phone is Busy, "If my phone is busy or unregistered, then:" Is there a way I can have one IVR set for 'busy' and another one for 'unregistered'?

The way the extension forwarding rules currently stand, there does not appear to be a way to differentiate between busy and not registered. You could always submit a feature suggestion for future versions. http://3cx.ideascale.com/

Will do. The 'unregistered' status could be used in case the Internet connection goes down, and 'busy' when the line is busy.

Any progress on getting busy separated from unregistered since it has been almost three years ago?

Unless there is a good reason to keep both together? Unregistered in particular is very important in an IP based phone system since Internet is the weakest link and needs to be addressed with basic fault tolerance options.
