So today none of my IVR extensions are registered in 3cx. The service is started and I've gone through the trouble shooting steps in http://www.3cx.com/blog/docs/troubleshooting-iis-ivr/ and need some help. When I try to call an IVR extension (like voicemail), I get a message that says "temporarily not available" on the phone.

I'm trying to check my logs and can't see anything obvious. The IVR service is running and IIS seems to be running fine as well. I reinstalled 3CX but I first backed up the configuration. I then restored the backup and still had the same issue.

The only change I did over the weekend was to test a VM on the machine with 3CX. It created a virtual network device with it's own IP which might have caused an issue? I have since removed the VM and the network adapter associated with it, but no luck there.

I should add that the 3CXIvrServer.exe process is now using a lot of CPU, nearly 100% of one thread.

    Any ideas? Any help?

I found this in the 3CXMediaServer.trace.log file:

    MediaServer.cpp(1724)|Trace5||??:call 1@127.0.0.1:5482 not found

Not sure if this is related.

I would suggest that there is something remaining, of this change. You may have to go in search of what did not return the machine to the previous configuration. It may, or may not, have something to do with audio drivers/devices.

    I have 2 servers. 1 v12.0 and v14
    I used v 12.0.34902.600 it's working well.
I ever updated to v12.5 is not working, and then I updated to v14 is working very well.

In the C:\ProgramData\3CX\Instance1\Data\Logs\ (or C:\ProgramData\3CX\Data\Logs\) folder, in the 3CXIvrServer.log do you see any line containing:
    Error ID 302. URLhttp://<ip address of server>/ivr/

If you take the underlined part and copy-paste into a browser on the 3CX Server, do you see a "greetings audio set to" result or an error?

This is the only error I see in 3CXIvrServer.log

    "Error! Module '3CX.com.OSBinet'. Error ID 303. "

It even says it successfully registered all the system extensions even though they are still showing red in the system extension status screen.

What about when you enter in the browser http://<LAN IP Address>:5000/ivr , not http://localhost:5000/ivr, do you still see the "greetings audio set to" message?

If you don't could you tell us what LAN IP address the server has?

Also, if you have not tried this yet, but it might be worth making a full backup and re-installing the 3CX Phone System software, maybe even consider trying out V14 as the installation process and the restoration process is a bit more 'intuitive'.

It says the same thing "greetings audio set to" when I go to the LAN ip: http://192.168.0.252:5000/ivr

I have already tried a backup uninstall, re-install and restore and the same issue exists. I don't have V14 and hadn't planned on upgrading.

I checked other phone systems and noticed that the 303 error appears on them as well with no issue on their end so right now I'm at a loss :( .

The "303 error" does indeed appear in the logs but that is to normal, it is not the reason you are having this issue. For the actual reason this is happening I am at a loss.
