I have a 3cx server running and can call between iOS devices that are connected internally. So I know we are "working".
I tested a Mac Softphone and audio travels both ways as well.

However, I can not get audio in the 3cx Phone for Windows. Even dialing 9999 for voicemail, it rings and connects, but no audio. (Windows 7)

Firewall if off on Client and Server.

All of this is internal comm only - nothing thru the corporate firewall (yet).

What does the 3CX log show? Any clue in there?
Have you tried this on any other PC, it could be an internal PC/audio issue.

Using ZOIPER softphone from my Mac I hear audio just fine.

Even better - the Softphone works fine when outside the firewall - tested on the same MacBookPro with Parallels Win 7 - disconnected from Wifi - plugged hard line into public side of our network and put in the 66.xx.xx.xx external IP. audio works fine.
So this is pretty strange to me.

On my Windows 7 Desktop machine - it was using "High Definition Audio Device" which was installed by default by Windows 7. It works fine for audio in general. 3CX Phone did not like it
I went to the Dell website and installed the Win 7 specific driver and now audio is working.

I am assuming similar problem with the Parallels Audio driver on my Mac.
