For the last few weeks, nearly all our incoming calls show the word "Octothorpe" on the caller ID, while the number is ok. Rarely the name will come in correctly.

    3CX version 8, 2x Patton SN4114, Grandstream GXP 2020 phones, United States

Is this message coming from the gateways or is 3CX generating it? Does anyone know how to fix it?

The 3CX logs will show if the word is being sent, along with the number, by your provider or Gateway. As you are using an FXO gateway, you can confirm what your PSTN provider is sending by attaching a Caller Id device in parallel with one of the phone lines and then call in. If that shows a proper name but the 3CX extension continues to show "Octothorp", then it is almost certain to be the Patton. Had there been any recent work or changes done to the system, especially near the time this began? Have you tried a power down on the Patton?
