When I check "PBX Delivers Audio" the audio quality becomes terrible. Our routers do not support full cone (Watchguard) so this is a must for remote extensions. When it is unchecked quality is very good, when it is checked it sounds like a low bit rate tin can...

    How can I fix this?

You are forcing the audio to route to and from the server...so, it could be putting a bit of a strain on your network and/or the server, depending on your configuration. It may be the Codecs that the sets are using, do you use G711, they tend require the least processing power. Do the logs show transcoding happening? Is this between remote extensions at the same location, or back to a set at the server location?
