    Greetings.

I am writing this time because I have the central point and confirguada perfectly with remote extensions without problem but when my iphone work perfect as local extention works, but when I'm remote or wifi or 3g I make calls simply by dialing the hang up immediately.

Any help or guidance would appreciate the advance.

I'm not positive, but it sounds like you are using the app as an extension off the 3CX PBX, correct?
If so, then check the 3CX log (verbose mode shows more information). There may be some clues as to why the call is dropping, or if it is even reaching the PBX.

I have a laptop configured remotely 3CXPhone and works perfect.

It looks like the call is being rejected because you are sending to a URL rather than the public IP of your router (that 3CX is behind). If you want to do that you have to put your DynDNS "name" into the Domain Name setting in 3CX.

leejor is right, if the IP you are coming from is not an IP the host is having it self (always the case in a NAT setting), we will match the domain name (settings advanced sip domain) with the one you have set at "External IP PBX" to be the same!
If not the same = forbidden...
