Problems with remote extention

We are having difficulities getting the 3cx softphone to work. It is showing in the system as 'registered' however no internal or external calls can be made, it keeps saying 'forbidden'. The box to allow use outside of the LAN is unchecked - any ideas? There is only one extention on the remote IP.
 
I would expect it to be a firewall issue on either end
Also check to make sure all the ports are being forwarded correctly on 3CX end (Router)
 
What do the 3CX server logs show? Any clues there? The "forbidden" message should not be a firewall issue.
