Voicemail Menu Appended to Voicemails

Have a slightly odd interment issue.

Users are complaining they're receiving voicemail's with some oddities in them. For example, the message will begin with the voicemail menu, silence for thirty or so seconds, the actual message, the voicemail menu again, and then silence for another thirty seconds. The activity logs show the call is from the voicemail ext to the users voicemail ext (ie, from VMail (999) to VMail (###)). Whereas if this were working properly it'd show from external number to VMail (###).

Checked the server and no event logs around the time of the message.

Server fully patched and restarted day prior to user's most recent complaint.

Any input would be greatly appreciated.
 
Have you asked a user who gets their vmail messages by email to send you an example of this ?
... could be a transfer gone wrong.
.. do these users have their vmail setup to login without a pin ?
 
I do have the vmail message they received by email, it's as described.

They do not have vmail setup to login without pin.

Have had no luck replicating the issue.
 
Hmmmm ... interesting.

I'm still betting on user error though. I've gone through something similar with a receptionist "transferring" a call direct to voice-mail, but doing it wrong ...
 
Or someone transferring a message from one mailbox to another (and again doing it very wrong)
 
... I'd love to hear an example ...
