    Comcast SIP works perfectly. Comcast has a setup document for 3CX and certifies 15.5 for use with their system. I have 24 sip lines with them since 3CX version 14.

    Check the Blacklisted IP's. The IP for that phone might be there as when you experimented with the extension it could no longer authenticate. After so many authentication failures, the IP gets blacklisted.

    Comfirming also that Comcast Enterprise SIP works great with 3CX, it is certified and supported in their system and they have extensive documentation available for setup. I have 32 lines SIP and Patton ATA for Fax. Works very well.
