3 way calling Softphone/Mobile App

Just wondering if anyone ran into this issue and has figured out a resolve. I know that the conferencing/three way calling works fine for the Deskphone. I have a couple users using their 3CX mobile app on their iPhones and sometimes they want to add a 3rd person to the call but they either get a call failure and when they hit conference the user on the other end hears the hold music for a second then goes back to the active call as if the conference just drops itself. Either way, am I missing something on what is needed from the PBX in order to handle these sort of calls with the softphone app? Any help would be appreciated.

On the deskphone is the person using the Windows/web client to conference or a button on the phone (which is probably making a second call)? In your outbound rule is the conference room extension (700) allowed to make outgoing calls?
