This is already resolved but thought I'd post it incase anyone has a similar issue in the future.
Presence wasn't working, so couldn't change status or see the presence of other extensions.
The issue was only happening on 1 machine, but the other 7 were fine. All running Windows 10 Pro.
Tried disabling Windows Firewall, AV and many other things on the local computer.
The problem was to do with the WatchGuard firewall filtering the traffic - ended up making a policy to not filter 3CX traffic and presence started working fine on that computer.
