Appliance working but can't get ports to pass

When running ports on appliance, all ports pass expect ALG, ports 5060 and 5061... the box is going through Sophos and ALG is disabled on it..

Must I also open ports on router as well?? as technically firewall connection test originated from inside LAN going OUT, not coming in

The way the firewall checker works will also cause traffic to come in on a different port and from a different IP so yes, you do need to configure your network firewall too to allow incoming traffic on all required ports: https://www.3cx.com/docs/ports/
