Auto Provision 3CXPhone

I'm trying to auto-provision a (windows) 3CXPhone, it shows up in Phones and ready for provisioning. However, if I select it then select Add Extension or Add Existing Extension, the form shows up, but when I click OK or APPLY, a popup shows up with this: "Please enter valid values for Model" and highlights the Model drop down on the Phone Provisioning tab. I do not see the 3CXPhone in this list to select. What should I select from that list for the 3CXPhone software phone?

have you tried using the autoprovision link in the 3cxphone that is sent out with the new suer email ?
this seems to work 100%.
