I can't initiate external calls from the 3cx client.
If I dial a number from my fanvil handset, it works.
But if I dial the exact same number from the 3cx client, it doesnt work.

Getting this in the logs...
21/06/2018 11:58:51 - Call to T:Line:10000>>*****@[Dev:sip:10000@*****:5060] from L:6229.3[Extn:282] failed, cause: Cause: 400 Bad Request/INVITE from *****:5060
21/06/2018 11:58:51 - [CM503003]: Call(C:6229): Call to <sip:*****@*****:5060> has failed; Cause: 400 Bad Request/INVITE from *****:5060
21/06/2018 11:58:50 - [CM503003]: Call(C:6229): Call to <sip:282@*****:5060> has failed; Cause: 487 Request Terminated/INVITE from 127.0.0.1:5488

Any ideas?

There is not much detail in the fragment you have sent however a 400 BAD REQUEST is normally returned when the request could not be understood by the server.

A 4xx is a client error so it must be something on the 3CX client itself - please confirm the state of the client (on hook, in CTI or softphone mode) whether you are using the 3CX tunnel to connect (port 5090 which is enabled by default) or whether you have turned it off.

Also the machine that you are making the call from, what OS is it running, what is the .NET framework on the machine and do you have anything like Windows firewall or anti-virus turned on.

Are your voice and data networks split with VLAN's ? if local calls work OK then this should be irrelevant but a little more understanding on the network and the call flow can sometimes be useful.

I am assuming of course it is a Windows based client and not Mac.

Additionally to what @eddv123 said, access your management console and navigate to Settings / Network / FQDN and make sure that your local SIP domain matches your FQDN.
