On version 18 I would go into C:\Program Files\3CX Phone System\Bin\nginx\conf\instance1 and update the acme.com-crt.pem and acme.com-key.pem files with the newly generate crt and key information and then restart the nginx server service. This seems to have changed with v20 as the above file location is empty so I tried going into Admin > Secure SIP and plugging in my crt and key info., which it appears to accepted/save, restarted nginx service but the cert expiration date still shows as today and not next year.

Why you didnt tried the "old" way? This didnt changed.

Yes, I tried the old way. This folder is empty, so I added the .crt and .key files but noticed there is not a dhparam.pem file. No clue how to create this file, it was always "there" and I never touched it. Restarted the nginx service but as mentioned above the date still shows as expiring today.

There should be 3 more folders... But this is the way i know how it works. Maybe someone else have an idea why it is not working for you

In version 20 it is in the "Program Data" folder

Thank you Nicholas for leading me in the right direction, finally got something working in v20.

For those who run into this same issue, the folder path is - C:\ProgramData\3CX\Bin\nginx\conf\Instance1

Rename your crt and key files as domain_cert_acme.com.pem and domain_key_acme.com.pem of course plug in your own FQDN. Then in services restart 3cx PhoneSystem Nginx Server serivce.
