OK I admit I missed that part, I'm aware that BT hardware is rarely seen outside the UK but I'm also aware that often if you peel back the label on ISP-specific routers you find a generic model or at least the same hardware and/or firmware may turn up in different boxes.

Also it didn't look like CGNAT, ( or walk like CGNAT or quack like CGNAT) it really had the look of a (cheap) router problem. Router not firewall as I can't believe a half-way decent firewall would have one-sided port forwarding, even most home routers I've seen do it right. Unless I don't understand CGNAT properly (quite likely) I think you'd have a hard time getting that many inbound ports open.

CGNAT allows ISPs to assign 1 IP to multiple devices giving them unique ports to use.

My bad I just reread the first post and saw that only 5060 and 5090 were tested, I thought the OP had just trimmed out the later tests, so I guess that's the result you'd expect to see for NAT whether CGNAT or otherwise as no other tests were shown.

What was the configuration screen shown in that screenshot? I was struggling to place the logo but its just MS copilot isn't it.

when using SIM Base routers (4G/5G) you are not holding any real public IP on your router, so you won;t be able to do the public IP.
you have to get the fiber Inter with PPPoE so that you will have real public IP on your router. May be Dynamic Public IP on your router which will change every week one time so still its fine for running 3CX.
