Internal calls getting blank sound

I have client running 3CX hosted with 3CX and there is an issue when they make internal calls. They are getting 3 seconds of blankness for both ends of the call - basically neither party can hear the other during the 3 seconds. This happens randomly during the call and can happen multiple times if they are on the call long enough. The call does not disconnect. I have spent hours on this with Sophos as there is a Sophos firewall on the premises. I have even taken the firewall out of the picture and just had a standard TP Link modem in as a replacement but the issue does not go away.
It does not happen with external calls. They have yealink T53W handsets and I am can't figure out if its a Yealink or a 3CX issue and I don't know how to figure out if its one or the other or both. I am hoping someone else has experienced this issue and might have a solution.

How are the handsets connected to the PBX? Is there an SBC? Is call recording or PBX DElivers Audio setting active? Can you packet capture on the phone or switch that the phones are connected on, to monitor their communication?

Calls from/to external numbers are good?
