I've sent logs and a bug report to Plenom on the subject and after some research they replied with "I have been talking with our developer, who tells me that the 3CX integrations unfortunately isn't able to handle multiple calls. He will, when he have time, investigate the options to support multiple calls."
Until changes are made to the integration between the busy light and 3CX, be aware that if you allow multiple simultaneous calls per extension, the light will switch back to green when any of them end rather than when all of them end.
