Solved No "Add Phone popup" deploying new extension in SP6 PBX

On a fresh install, we are unable to add phones to extensions: under extensions -> phone provisioning -> add phone -> (nothing happens). It appears to be a problem with the Add Phone popup, however, we have used multiple workstations - firefox, chrome, IE - cleared caches, cookies, etc. and still no luck. Vanilla Linux installation passes firewall test, etc.

Anybody have any ideas, suggestions?

Can you add a phone from the phones page?

Is the issue present to all extensions? Did you try to create a new extension and see if that works? Do you get an error when you try to add a phone or an exception?

Also please navigate to Settings / Templates and check that phone templates are present.
 
Apparently, this was a bad install. Since it was a new PBX, we backed it up, deleted the instance, built a new one (yay Debian), and did a restore. The problem went away. We setup several PBX's every day and we've never run into this kind of issue. In retrospect, wish we'd kept the instance to identify where the issue was. But, as with the creepy spider on the keyboard, it got smashed and trashed. I'm thinking a library install didn't happen and we missed the error.
 
Glad to see that issue has been resolved and thank you for updating the thread with your solution. It would have been interesting to see what caused the issue as we have never come across this either.
