One single beep when busy

I noticed that sometimes I get a normal busy tone when the callee is busy: beep beep beep beep
Sometimes I get only one single beep and on my phone: please hang up.

What is that? My users think my PBX is out of order...

What types of calls are these? Outside, internal, different behaviour to the same number?

They are outside calls. And it is strange, because the reply code is always 486:
Call or Registration to 0571646xxx@(Ln.10004@QSC Kiel) has failed. 213.148.136.222 replied: 486 Busy Here; warning: SoftX3000 R601-CCU Rel POS:[3103] Release from CR; from IP:213.148.136.222:5060

Since you are getting that reply from your provider, you may want to question them about it. Depending on how helpful they are, they may be willing to examine your call attempt, from their point of view, to determine why you are getting that log.
