Phone keeps ringing reapeatedly, is this a hacker?

I am frequently having a problem (once or twice per day) where one of our SIP phones (both hardware and software) will ring and when I answer the call no one is there. When I hang up it immediately starts ringing and does not stop no matter how many times I answer and hang up. I inevitably unplug the phone or quit the softphone app to get it to stop.

Is this an indication that someone from the outside is trying to gain access to our system or is this some weird Elastix glitch?
 
It could be attempts to compromise your system, you need to check your asterisk logs and look for the attempts

Just have a look at the following logs

/var/log/secure(.x) - .x being the log rotation

and

/var/log/asterisk/full(.x) - as above.

In particular look at the times that the calls were made (otherwise you could be looking for ages)

Also look in the /var/log/asterisk/full logs for any phone (re-)registrations a short while before this....

Also do you have anonymous SIP turned on....it could be someone probing your system. If you have the capability, with your router/firewall, limit where you SIP ports will allow traffic from (e.g. your SIP provider).....
