Provisioning Polycom VVX 500

Hi, has anyone successfully provisioned Polycom VVX500? I followed the provisioning guide for Polycom phone, but no success. I also tried manual provision, but not sure what to enter for the Provisioning Server field in phone's web GUI. Please advice. Thanks!!
