Recurring error message

I get the following 2 error messages multiple times each day.

Call or Registration to 4@(Ln.10000@Vxxxty) failed.
sip:11.22.33.44.;lr replied: Address Incomplete (484)

Call or Registration to 4@(Ln.10002@IXXXA IP Trunk) failed.
sip:111.22.333.44:5060;lr replied: Forbidden (403)

If I look at the call log I see the following:

I can find no place in call routing, call groups etc. where this phantom "4" is coming from. Is there someplace else I should be looking?

Four Months later I am still trying to resolve this issue. Does anyone have any idea where I should be looking? I can't believe that I am unable to find the source of the issue.
