Strange noise when placing outbound call

We have t42s phones making a loud squeal randomly on outbound calls almost immediately. Maybe once or twice a day on a few different phones at two different sites. Has anyone else experienced this?

What version. If 20 try using the call monitor function.
Firewall passes?. 3cx and phones up to date?
