Strange One Way Audio Issue

Here is the scenario:

We have a working production on-premise v18 with external Fanvil X3U phones connected to our LAN through VPN on the phone to an internal VPN server. These phones communicate with 3CX internally and everything works as expected.

We have setup a new v20 on-premise PBX and are attempting to re-create the existing v18 setup for another organization, including a separate on-premise internal VPN server. I have matched the configurations and have triple checked everything I can think of but we are only getting RTP packets in one direction on the new setup.

Phones connected to local LAN (no VPN) are working for incoming and outgoing for internal and external calls. Once I turn VPN on in the phone I only have outgoing audio and no incoming audio from the PBX, echo test also has no audio.

Firewall checker passes.
I've matched all NAT rules against the working v18 setup.
I've tried split DNS and Hairpin NAT
I've tried using internal IP and also the FQDN

One thing that is interesting is that a packet capture on a working v18 phone when calling voicemail shows the RTP packets originating from the PBX while doing the same on the non-working v20 phone shows the RTP originating from the phone IP.
