I can not find any way to change the behavior of these, I would like to list both the IP for internal access (or a different FQDN) and the external FQDN for access from out of the office. The template does not seem to actually offer this option tho.
 
Why do you need the internal IP on these?

Just let everyone use the external FQDN and setup your internal DNS to redirect as needed.
 
We can't use SplitDNS because VPN Users are connected to the internal DNS, but shouldn't connect to the PBX via internal IP.

Because he doesn't want to encrypt the audio, adding load to the VPN.
I'm assuming an SSLVPN on the client.
If the client is ALWAYS external. you can add the 3cx outside IP to the Host file with the external address.

This won't work if the client moves internal/external. In this case you could maintain 2 different internal DNS servers (a real pain but will work). One server is your true internal DNS and it is given out via dhcp to local clients. Then have the sslvpn gateway hand out the other DNS server to remote clients, on the sslvpn DNS server manually setup the 3cx host to point to the external address, you will need to manually configure all the other host on the second server as well.
