3cx call log shows vmail calling extensions

I have a client who is getting voicemails with the caller ID of the voicemail extension 9999. When I check the call log I see multiple calls from vmail 9999 calling other extensions then showing it left a voicemail lasting over 2 minutes with the digital receptionist repeating the voicemail options then 1 min into the voicemail is an actual caller leaving a vm . Any idea why this is happening. I support over 20 3cx systems and have never seen this behavior before.

There's a number of reasons why this could be happening. You'll need information from the activity log, preferably in verbose mode, and not the call log to determine why.
 
As @cobaltit mentioned you will need the Activity log to be able to determine what is causing this. You will need to follow the call route and see why it ends up in voicemail.
