Have you successfully run the 3CX Firewall Checker? All ports? What type of router/firewall are you using, as this..

some of SIP/SDP headers may contain inconsistent information or modified by intermediate hop SIP proxy detected:

.. is a bit concerning.

You seem to indicate that this is has been working for a while. That being the case...something must have changed, somewhere. Things don't just fail, for no reason.
 
Agreed...trying to find anything on my network that changed/possibly upgraded in the last 2 day. Unifi USG 4P. Full firewall test passed.

Have you tried a process of elimination, and bypassed the Unifi, as a test?

Make a new, fresh extension. Log it into the webclient. Don't change anything.

Then edit the trunk settings, send calls for in and out of office hours to that extension directly.

Will it ring now?
 
These 169 addresses aren't on my network. Could these be the inbound failure/ loop?
You retrieve an address in APIPA (Windows DHCP), address that is retrieved when your NIC does not find any IP address assignment service.

You can try this
In the Windows menu, All programs, Accessories you right click on Command Prompt and in the menu on Run as administrator. A DOS type window opens.
At the prompt, you type ipconfig / flushdns
You click on Enter and you start again with the following ones.
netsh winsock reset
netsh winhttp reset proxy
netsh winhttp reset tracing
netsh winsock reset catalog
netsh int ipv4 reset catalog
netsh int ipv6 reset catalog
