Accept multiple calls is disabled
Calls are coming in direct to the extension via DID.
Not every call does this; some do.

Anyone have any ideas?

Are you using the webclient as a stand alone client or are you controlling an IP phone? Also what browser are you using?

We are using this on Google Chrome and controlling a Yealink T46S.

What firmware is your Yealink running and are you using the default template or a custom one?
