3CX 16.0.8 and 16.0.9

I'm following this guide:
https://www.3cx.com/sip-phones/avaya-ip-phone-provisioning/
My first concern is that guide appears to be stale; it says to download template, provisioning files, and logos. The template and logos are already present by default in the folders I'm instructed to place them.
I extracted the provisioning files into the [secure_folder] as instructed, which felt weird, because that directory is just a mess of loose files, and the zip had a lot of stuff in it!

The DHCP stuff goes fine, and then phone reaches for a file called `96x1Supgrade.txt` from the HTTP server. This file doesn't exist, and it fails with 404.
It doesn't exist in the provisioning package I was instructed to download... where is it supposed to come from? Perhaps the phone shouldn't be looking for this file, and something else is wrong?

Has anyone successfully provisioned these phones?

I'm also concerned about this note in the guide:

Code:
Limitations and known issues
The following limitations apply to these phones:

* Extension number and Authentication ID must be the same.
...
That doesn't seem to be possible, because the 3CX UI complains that `Authentication ID` is too short... so, that's an oversight! Am I doomed to fail, even I resolve the 404 step where I'm stuck?

Here's a weird observation, in the provisioning xml I was instructed to download, there's a sectrion:
Code:
<data>
    <device>
      <type>phone</type>
      <!-- Friendly Name -->
      <field name="Name">AvayaUpgrade</field>
      <deviceconfig filename="96x1Supgrade.txt">
<![CDATA[
...
the file
...
]]>
</deviceconfig>
    </device>
  </data>
It seems there's a copy of the missing file hidden in the provisioning xml... but the phone is looking for it on the http server... how is it supposed to find the file here? There must be some magic performed by 3CX, which isn't working right...?

So... what I did, was I copied the "96x1Supgrade.txt" text from `avaya.ph.xml`, and I pasted it into a file named `96x1Supgrade.txt`... the phone found that file and moved on, then it looked for `46xxsettings.txt` and 404-ed again, which is also another block of text from the XML file.
I created that file aswell... the phone continued, found the firmware, copied it to the device, flashed the software, reboot and finally prompted me for username and password, which are the auth ID + password from the 'provisioning' tab in your extension settings.
The phone works now.

I never saw those steps noted in any guides... why were they necessary for me? Why did the guides not work? What's the story with embedding those files into the provisioning xml file? How was the phone ever meant to find them there?
These mysteries could use explanations if anyone knows what they're talking about?

This may be related to the firmware the device was running prior to upgrading it. As mentioned in the guide, "Phones running on H323 Firmware must be converted to SIP prior to provisioning".

The vendor may be able to shed some light on the matter so you might want to consider contacting them.

Just bear in mind that these phones are now EoL so their support is limited plus they will not be receiving further development, I'd recommend slowly transitioning over to other 3CX Supported devices/models: https://www.3cx.com/sip-phones/

I set-up our Avaya phones using DHCP and HTTP
