Call Dropped On Second Transfer

One of my clients is having an unusual problem with transferring calls and I'd appreciate any help people can provide.

I am running 3CX v 16.0.910 on a Windows server.

This Works
When a call comes in to reception it can be transferred to another extension without being dropped.

This Doesn't Work!

1. Call comes in to reception extension
2. Reception tries to transfer the call to another extension that is busy or on DND
3. The call comes back to reception
4. Reception tries to transfer the call to a different extension. The call gets dropped by the system at this point!

I have enabled "PBX Delivers Audio" on the reception extension (it wasn't enabled before) to see if this helps resolve the issue but any help you can provide would be greatly appreciated.
