What is the preferred way to control outgoing call restrictions. Have the user enter a code to make an international call so it can be identified who is doing so.

You could do this in a number of ways depending on the situation. If it were only several extensions that require access to international, then an outbound rule could be created that was only used by those extensions. Anyone use those extensions, could then place calls to the destinations allowed in the rule(s). If you want to restrict even further, you could create a rule, not only limited to one or more extensions, but one that required a prefix, that would be stripped before the number, is sent out on the trunk. This could be 5 or 6 digits long, and unique to each person. Of course a rule would have to be created for each unique prefix. This could be limited to one set, or allow that person to use any set in the company. all depends how you set up the outbound rules. Don't forget they are "read" top to bottom, so first match, is used.

Thank you. Do you know if you use the prefix option, if there would be a way in call reports to determine who made these types of calls and the details of them? Quantity, duration, etc?

Running a call report should give you all the call details you need. You can filter with outbound calls only and with the required extensions. If you need to filter only with calls made internationally you can use the CDR data and filter results manually to get the required information.
