I have a customer that has remote locations running on the same 3cx. When each city is called it rings to a ring group and transfers to a external number if the call isn't answered. The call isn't sending the correct caller id from each location, but sending the caller id of the location that it's ringing to. It's sending the default caller id of the sip trunk.

To be clear...you want the original callers number to show, but are getting the caller ID of the outbound trunk, or of ...what? You may need to be a bit more specific. Given the remote location is running on the one 3CX, I have to assume that the calls route out on trunks from the PBX location. Do you use this same scenario with a ring group that is not "remote", and that does function the way you expect it to? Does your provider allow the originators caller ID to be sent, on a forwarded call...not a number acquired from them?
 
In the outbound trunk parameters, one of the options, probably needs to be changed to OriginatorCallerID.

..and keep in mind that the options vary from provider to provider. Do a screen print of the current settings before making changes.
 
Check that calls from an extension still show the correct DID when calling out. ...and that calls
from an extension without an outside caller ID data-filled will show the main trunk number.

The calls ring into a ring group for 20 seconds and then they go to a external number that's not on the 3cx. The caller id they see is the caller id on the sip trunk on the 3cx not the caller's number

Does your provider allow you to send a caller ID other than your main trunk number? Do DID numbers, on outbound calls made by extension show correctly? Do the originators numbers show correctly(on a mobile) if made to an extension, that has the ring my mobile at the same time option enabled?
 
Now, does anyone know of a possible downside to this? There must be a reason they were set differently?
If you provider does not support Clip no screening (sending the original number) then calls might be dropped by the provider.

To be able to send the originator caller id you need to make sure that your provider supports this function. Then you need to ask your provider what is the sip field the expect this information to. Once you have the necessary information you can then edit your trunks outbound parameters and set "Originator Caller ID" to the correct sip field.
 
Since you are an ITSP then you should be aware of the correct settings needed and perform the necessary changes in the outbound parameters of the PBX. The 3CX PBX can be setup to send whatever you need it to send. If your platform accepts the setup then it should be fine for all calls.
