Caller "lost" ( with Hold-On Music ) after a Consultative Transfer

I have a sporatic issue with the consultative transfer.

And the end of a consultative transfer the callee doesn't hear anything, and the caller still hear the hold on music.
If the callee hangs , the call is ended normaly .

Look at this example :

20/08/2019 12:18:51 (Caller) EXT (324) 00:00:17
20/08/2019 12:18:47 EXT (340) EXT (324) 00:00:04
20/08/2019 12:17:55 (Caller) EXT (340)

Extension 324 doesn't hear the Caller at 12:18:51 , and the caller still hear the hold-on music during the 17 seconds

Any idea ?

Here is a Response from our Fantastic VOIP Provider (Wiretap Telecom) that has, so far, alleviated our issues.. Not sure if this would fix others issues in this forum. I will continue to monitor.. but here is his message:

We installed a minor release this weekend that might have resolved your issue. As I read the release notes I see that there was a fix for when a trunk receives an automated call and the end-user presses # to acknowledge the call and to interact with the other system. This might seem unrelated to your issue, but under some circumstances our systems would intervene and process the # key as a forward request. This was once a desired feature for a product we no longer offer. This feature has been removed from our switching systems.

In other words, this update removed call transfer logic for trunks so it is very possible that this indirectly solved your issue as well.
