Cannot import bulk extension in 16.0.6.641

Hello all,
After update 3cx to 16.0.6.641 it is impossible to import extensions with CSV.
We receive the following error:
An item with the same key has already been added. Key: yealinkT4x.ph.xml_Yealink T19P E2

We tried the following scenarious:
1. Create some dummy extension, export it to csv, delete the dummy extension and import the previously exported csv.
2. Export all current extensons to csv, delete all the extensions and import them again.

In both cases we received the mentioned error.
Please advice.

Fixed - we had a custom template in
/var/lib/3cxpbx/Instance1/Data/Http/Interface/provisioning/#someinstancenumber/CustomTemplates/phones/
