Cant call most 800 #'s and local numbers

Outbound calls to 800 numbers are not working. we get an out of service message. Calls to local numbers are not working. We here a busy tone before the call ends. I will list the activity from the errors below.Thank you all!

Also i would remove any private into from your log such as FQDN/IP addresses.

How is the authentication setup on your sip?

One other thing to check is whether the provider allows calls to those destinations (assuming that your outbound rules allow you to even send the call out).

You may have to manage the DIDs from the flowroute portal and see if there are restrictions.

In addition, if you are calling an 800 number, why do we see this other number instead?

In the second section (calls to 800 numbers), you've listed attempts to 3 digit numbers, not 800 numbers. Outbound rule issue? For calls to the 760 number, the response (403) from the provider, would seem to indicate that the Authentication Method you are currently using, is not correct.
