cant make calls from external VoIP Phone

i can't make calls or receive calls from an external ip. I have every port open in tcp and udp. the phone registers but it wont make the calls. this same phone works when on my internal network.

Did you enabled STUN on the phone when you moved it off of the local LAN. You need to. What does the registration or a call attempt look like in the 3CX logs?

I should probably make this clear, the calls i am trying to make are between extensions. because I don't have a PTSN device yet

what should my STUN settings look like?

it also fails when i am using the server domain (http://www.mydomain.com) instead of the local ip (192.168.xxx.xxx) on my phone
