Can't make outgoing calls

I just setup our 3cx system, and everything seems to work perfectly with the exception of outgoing calls. Here's our setup:
Grandstream 4104 connected to 2 PSTN lines
5 Linksys SPA942 phones
Incoming calls work great, paging, voicemail etc.
I've setup an outbound rule that has the grandstream in route 1, strip digits 1, nothing in prepend, and the Prefix is set to 9. I just get a fast busy when I try to dial out. The server activity log says "Normal call termination. Reason: Forbidden"
I went throught the manual and configuration guides, I can't find anything wrong. Any suggestions?
 
Try leaving the prefix empty
Add the extensions you are using in the specified area
I use 7-18 digit code so where it asks number length just put '7-18'

I have my first route setup like this.

Route 1: VoIP Provider | Strip Digits: 0 | Prepend: (Insert Area Code)
Route 2: VoIP provider | Strip Digits: 0 | Prepend: (Empty)
 
Please post the 3CX log of a failed call.
 
After much troubleshooting, I finally figured it out, and I don't know why this makes a difference, but the FXO gateway had no entries for port 3 and 4 since we only have 2 incoming lines, after adding entries for port 3 and 4 and configuring 3cx to not accept incoming or outgoing calls on those 2 lines everything worked perfectly. Thanks for your suggestions.
