Ok so I realize that on outbound calls you can set the caller ID to whatever you want at the extension level and it'll pass through. What if the number you're "spoofing" is CNAM registered? Will the spoofed called also show the name on the caller ID?

My 2nd question pertains to inbound calls. Do I need to setup a unique sip trunk connection in 3cx for each DID? I'm a little confused how I'd manage inbound calls otherwise. Also, is it possible for a softphone to be able to receive calls from multiple DIDs I own at the same time?

Reg 1: CNAM resolution is made by intermediate hops, so if they see caller ID A then they will show the name of it.

Reg 2: I would advice you to check the admin guide and our academy reg. this

Different end provider handle name display in different ways. In some cases, if a name is sent from the originator, the end provider will pass that name on. In other cases, a look-up will be performed, and a new name , found in the database, will be attached. It will differ from provider to provider.
