CRM Integration Problem

Hi, one of our customers has some problem with the CRM integration...
Using 3cx template generator he's getting some issues with the contact id, it seems not returning at all in the CDR...
Can someone send us a complete example of the XML configuration?
The attached file is his current configuration.

You can check the built-in templates to compare. My guess is the EntityId and EntityType are not returned in the lookup scenario.
