It would be a very good idea for 3CX - at least for US installations - to consider creating a default 911 outbound dial rule during the initial installation. At minimum it would help remind the installer that the dial rule needs to be matched up with a provider.

One of the problems with providing a "default" dialplan of any sort is that everyone has their own way of outbound dialling. Some may want you to dial 911, others 9+911, (as they have it set that all outbound calls need a 9 first) . Some may want users to dial 8+911. North America is just one market where 3CX is used, what about 112 for Europe or other emergency numbers in other countries. If you start to do one then you've opened the door to more requests. Not every user is using outgoing VoIP trunks , some may just have gateways to the PSTN. I guess it's up to the 3CX people.
