DND turns on by itself, can only be disabled with *60 dial code

We're running 3CX on Debian OS. Latest service pack, fully updated, phones are on the latest supported firmware.

I'm having an issue at a client where a phone will somehow get into DND status and then get stuck. I can log in to the affected extension via webclient and toggle the status there and the status will update but the extension stays in DND.

The only way I've found to resolve this is to open the extension in the 3CX Windows client and dial *60.

A different but likely related issue that happened when I had a reliable tech on-site. One of our extensions got a system generated call toggling DND status (it gave the announcement as though the caller dialed *32 to set the status) and then another call right after that turned it off.

So I'm having a couple of issues here. One, extensions are going DND without user interaction. Two, when this happens (but not every time) DND gets stuck on until I can open 3CXPhone and dial the DND dialcode of *60.
 
Have you checked the autoswitch settings for the extension

This setting is not enabled. It also wouldn't explain why DND turns on by itself in the middle of the day
 
*60 and *30 are different beasts! (unless you changed dial codes)
Can't say why it's turning itself on... but *3(x) will not affect a *6(x).

Any custom templates?
 
I haven't changed the dial codes around. I bring up the *3X dial codes because sometimes the affected phones seem to go into that style DND and *30 is the only way to get them out of it. And I'm also puzzled as to why one of my extensions received a call from the system (logs say it was from IVR:Make Call or something along those lines) to toggle the profile status and this wasn't triggered from the admin or the user side

So I'm really at a loss because its not just one type of DND turning on and the fix isn't always the same.

No custom templates. We did at one time but everything has been moved to the standard templates and been reprovisioned.
 
What phones are you using and are you using them along windows clients with CTI?
 
Mostly Yealink T42s with a few T46s. The end users don't use either the webclient or the windows client. The affected extensions are all in a couple of chained ring groups that take all the inbound calls for their practice. When they leave for the day, they put all the extensions in the group into DND so that calls go to a voicemail.
 
From your description i cannot think on anything that could cause the PBX to set the phones to DND. You will need to dive into the 3Cx Bin log viewer to determine what is causing this behaviour.
