Duplicate Entries in Extension Report

We are pulling monthly extension reports and seem to have duplicate entries. Users are listed twice. The first line is Lastname, Firstname. The next line is the same user, only Firstname Lastname. This doesn't happen for every user. If there is a duplicate, there is no data for that line. Is anyone else running into this?

We are on v20 Update 5.
