Have 3CX v16, Patton 4141, and Yealink phones

When we receive external calls for the first 10 seconds or so of the call there is an echo of our voice, as mentioned it dissipates after about 10 seconds. Outgoing calls are fine and internal calls are perfect.

On the Patton we have echo cancelling enabled with Hybrid Loss at 3 and Nonlinear processing on adaptive.

Any feedback on what may be causing this or some settings to try and change would be great.

It sounds (no pun intended) to be your echo suppressor adjusting. If you hear echo, it usually means that your outgoing level is too hot. This can be a result of the type of PSTN lines connected, distance from the central office, or a remote you may be connected to. You might try adding a few dBs outgoing padding, as a test.
